This packet was created in an effort to ensure that all staff members are provided with necessary information and are made aware of district procedures. Please take time to carefully review this packet, as you are responsible for this information. Enclosed are the following:
Title IX/CH. 622 Statement
The district is mandated by law to provide each staff member with this statement on a yearly basis.

North Adams Public Schools Payroll Calendar
The enclosed calendar indicates dates on which timesheets are due and dates on which paychecks are issued.
Memorandum Regarding Purchasing and Reimbursement
Please carefully read this memorandum. All faculty and staff members are responsible for this information and are expected to adhere to these procedures.
Conference Attendance Request, Course Approval Form, Field Trip Request Form
These forms are available on the district website. Please make sure you complete all the necessary information and submit necessary attachments.
Travel Expense Voucher
Be sure to attach itemized receipts when submitting expenses for reimbursement. You can not be reimbursed for gratuities or alcohol.
Student Accident Report
This form should be completed and submitted to your principal for signature on the same day which an accident occurs. The form should then be forwarded to the Superintendent’s Office. Please note this form is for student accidents only. Faculty and Staff accidents should be reported on the accident form available at your building office.
Report of Child(ren) Alleged to be Suffering from Serious Physical or Emotional Injury by Abuse or Neglect.
All faculty and staff members are mandated reporters and must report any allegation of serious physical or emotional injury resulting from abuse or neglect to the Department of Social Services. When completing this form, a copy needs to be submitted to the Superintendent. Please review the enclosed form and contact your School Adjustment Counselor for additional information.
Payroll Information
JoAnn Glogoski, Payroll Clerk, has provided some general information about various payroll issues. Included are memos regarding new health insurance rates and changes regarding 403(b) plans. All employees are strongly advised to take advantage of the direct deposit service. If you are interested in signing up for direct deposit, complete the enclosed form and attach a voided check or deposit slip and send it to JoAnn Glogowski in Central Office.
Special Education Referral Process
Please refer to this information regarding the special education referral process. If you have any questions, contact Noreen Donnelly, Administrator of Special Education at 662-3232.
District Curriculum Accommodation Plan
The District Curriculum Accommodation Plan can be viewed and downloaded from the district web site. It is necessary for all faculty and staff to familiarize themselves with this document.
