The Massachusetts Department of Public Health has requested that clinics be held throughout Massachusetts to try to provide protection from the flu to as many people as possible. Locally, Northern Berkshire Pandemic Planning Group will be conducting flu clinics in area schools.
The H1N1 Flu vaccination clinic will be conducted by members of the Planning Group and their staff. Individuals in the group are members of local boards of health, health departments, nurses and staff of VNA, area hospitals and/or physicians offices.
If you decide to take advantage of the School Flu Clinic, and have your child vaccinated in school, we ask you to please:
1. read the entire consent form,
2. answer each question, and
3. sign and date the consent form in the lower left corner.
If any question is not answered and/or the form is not signed, your child can not be vaccinated in school
You may also decide not to have your child vaccinated in school but rather go to your family doctor or attend one of the upcoming General Clinics. In this case we would ask that you sign and date the form in the lower right corner "I DO NOT GIVE CONSENT."
